That slab of concrete on top is the only thing keeping water out of the brick. A lot of them were poured thin, flush with the edge, with nothing to throw the water clear. Why the stain shows up inside
Water that gets in at the top doesn't stay there. It runs down inside the brick and comes out where the chimney meets the ceiling or the wall next to the fireplace. By the time you see it inside, it's usually been coming in for a while.
